to designing a plan that will assist in preventing relapse for
each patient. It is the patient’s responsibility to follow it.
Recommendations may include, but will not be limited to, attendance
at our outpatient addiction treatment center, individual therapy,
family therapy, a referral to address a specific issue, or halfway
house. Every addiction treatment plan is different, as is every
patients needs. The element every plan has in common is the
shared experience of long-term addiction recovery.
